SECOND PASSPORT CHECKLIST

* Complete and print 1 copy (Single-Sided Only) of the Online Government Passport Application Form DS-82.
* Hand-written applications are NOT accepted.
* Ensure bar codes (DS-82 B) are located at the bottom right hand corner of pages 1 & 2.

* Color photo must be 2 x 2 inches and have a white background and be professionally taken within the past 6 months.
* No uniforms or headgear, as well as, no white shirt.
* Eyeglasses are no longer allowed to be worn in passport photos.
* We recommend using the Passport Photo Creator mobile app. You will then pick up your photos at your nearest Walgreens location.

* Passport must be in good condition and have been issued for 10 years. It will not be accepted if damaged or mutilated.
* If your name has changed since the issuance of your current passport, an original or state certified name change document is required.

* If you are a frequent traveler, include a letter stating the reason why you need a second valid passport.

* The amount of the check will be either $190 and must be payable to the "US Department of State".
* If using a personal check, a pre-printed Name and Address must be located in the upper left hand corner. It cannot be a starter or temporary check.
* Ensure that the applicant's full name and date of birth is written in the memo section of the check or money order.

* Most commonly used itineraries are airline, cruise & hotel reservations.
* A Business Letter on Original Company Letterhead can be used in lieu of travel itinerary.
* The Travel Itinerary and DS-82 application dates must match and include the applicant's name.

* This Letter gives RushMyPassport permission to represent you at the U.S. Department of State.
* Ensure that the Courier Company Name is printed as it appears at the top of that page. This will help avoid processing delays. Hand print and sign in BLACK INK ONLY. your order without this Letter of Authorization.

* The Pre-paid Overnight Shipping Label is included in the Step 2 email, along with the Letter of Authorization form.
* Print this label and place on the outside of an overnight shipping envelope.
